For outpatient Evaluation and Management (E/M) services, “medical necessity” is ultimately judged against Medicare coverage policy, which is established through National Coverage Determinations (NCDs) and Local Coverage Determinations (LCDs). NCDs set nationwide rules for when specific services are considered reasonable and necessary, while LCDs are developed by Medicare Administrative Contractors to define coverage expectations within their jurisdictions, including indications, frequency limits, required documentation elements, and diagnosis-to-service relationships. Outpatient CDI and coding education emphasizes that correct E/M code selection and documentation must support not only the level of service (MDM/time) but also why the service was needed based on the patient’s condition and the payer’s coverage criteria. Specialty society recommendations and AMA/AHA guidance can inform clinical practice and coding conventions, but they do not define Medicare coverage requirements. Likewise, AHIMA provides professional guidance but does not set payer medical necessity policy. Therefore, the most accurate source defining medical necessity criteria for billing compliance is NCDs and LCDs.

A leading query is one that steers the provider toward a particular diagnosis or limits clinically appropriate choices in a way that can be perceived as prompting. Option D is leading because it presents a single, high-impact diagnosis (“alcohol dependence”) and forces a binary yes/no response without offering reasonable alternative interpretations (e.g., alcohol use, alcohol abuse/harmful use, dependence in remission, or clinically undetermined) or an “other” option. In addition, it attempts to obtain a potentially new diagnosis based on one data point (quantity consumed) without a balanced set of diagnostic possibilities and supporting clinical indicators (tolerance, withdrawal, impairment, failed attempts to cut down, etc.). By contrast, A is open-ended and requests clarification of the treated condition; B provides two plausible classification choices (active vs history); and C offers multiple reasonable BMI-related diagnostic options plus “other” and “clinically undetermined,” which supports compliant, non-leading clarification. Therefore, D best fits the definition of a leading query.

Under Medicare, OPPS is the payment system used primarily forhospital outpatient department (HOPD)services paid under APCs, and it also applies to a limited set ofnon-hospital entitiesfor specific covered services. Community Mental Health Centers (CMHCs) are included under OPPSfor certain outpatient mental health services, most notably partial hospitalization–type services that are paid using OPPS methodology, which is why CMHCs are considered “covered under OPPS” in many outpatient CDI education materials. In contrast,clinical diagnostic laboratory servicesare generallyexcluded from OPPSand paid under the Clinical Laboratory Fee Schedule (with separate billing and payment rules).Indian Health Servicesfollow different statutory and payment structures and are not paid broadly under OPPS in the same way as HOPDs/CMHC OPPS services.Physical therapyreimbursement is typically governed by therapy-specific rules and fee schedule methodologies rather than being a standard OPPS-covered category in this context. Therefore, among the listed options, CMHCs are the correct OPPS-covered selection.

Symbicort is an inhaled combination medication containing an inhaled corticosteroid (ICS) and a long-acting beta-agonist (LABA). Symbicort is not used to treat musculoskeletal degenerative disease (osteoarthritis), peripheral nerve pain from diabetes (diabetic neuropathy), or cardiac pump failure (congestive heart failure). When Symbicort appears on the active med list, outpatient CDI commonly checks that the provider’s documentation appropriately reflects asthma classification (intermittent vs persistent), current control, exacerbation status if applicable, and that the condition is being monitored/assessed/treated during the encounter to support reportability and accurate coding.

Chronic kidney disease (CKD) is defined by evidence of kidney damage or reduced kidney function that persists for at least three months. An estimated glomerular filtration rate (eGFR/GFR)below 60 mL/minsustained over that timeframe is an objective indicator of chronically decreased renal function and supports CKD identification and staging in the outpatient record. This is why outpatient CDI programs frequently use trended eGFR as a clinical indicator to prompt documentation of CKD stage (e.g., stage 3a/3b, stage 4, etc.) when appropriate. BNP >1000 is more aligned with heart failure severity/volume status rather than kidney damage. BUN <12 is within/near normal and does not indicate renal impairment (elevated BUN may be seen with renal dysfunction but is less specific and affected by hydration, diet, GI bleed). Glucose >100 is a screening indicator for impaired fasting glucose/prediabetes but does not, by itself, establish chronic kidney damage. Therefore, sustained GFR <60 is the best objective lab-based measure of chronic kidney damage over time.

A well-maintained problem list supports continuity of care by giving the care team an accurate, up-to-date clinical “snapshot” of active and relevant historical conditions that affect ongoing management, decision-making, and risk assessment. Outpatient CDI education emphasizes that the problem list should be curated—conditions should be current, clinically meaningful, and appropriately resolved or clarified (e.g., active vs history, controlled vs uncontrolled). Option A is incorrect because diagnoses are not removed based on an arbitrary time threshold; they are updated based on clinical status (resolved, inactive, erroneous, or no longer relevant). Option C is inaccurate because simply adding more diagnoses can introduce noise and increase the risk of outdated or incorrect conditions being propagated (“problem list bloat”), which can harm patient safety and lead to inaccurate coding. Option D is inaccurate because CDI professionals typicallydo notindependently update the problem list; rather, they support providers through compliant queries, education, and process improvements so the treating provider validates and maintains the record. From an ACDIS outpatient CDI perspective, prospective querying improves efficiency and reduces retrospective “chart-chasing,” late addenda, and documentation gaps that occur when clarification is requested after the encounter is closed. Importantly, prospective queries must remain non-leading and cannot direct the provider to a particular diagnosis (eliminating option A). They also cannot “guarantee” risk-adjusted capture because the diagnosis must be clinically supported and addressed (eliminating option B). Defining the purpose of the encounter is driven by the clinical reason for visit, not by CDI (eliminating option D).

In outpatient CDI and coding education, selecting the correct E/M code starts with identifying theencounter category(e.g., office/outpatient vs inpatient/observation vs ED) and whether the patient isnew or established, because these define the applicable CPT code range. Next, thelevel of serviceis selected within that range based on the documentation supporting the required elements for that code family. For most E/M services, “site of service” (place/setting) and “new vs established” are foundational code-selection drivers, while “level” is determined by the record’s support for the applicable leveling methodology (commonly medical decision making and, when allowed/appropriate, time). Time can be a valid leveling method for many office/outpatient E/M visits, but it is not universally the basis for the majority of E/M services across all categories; it is an alternative pathway when documentation supports it. Physician specialty and patient age do not define the majority of E/M code selection. Therefore, the best overall statement is new/established status + site of service + level of service.

Risk adjustment is used to make cost and efficiency comparisons fair by accounting for differences in patient severity and expected resource needs. In outpatient CDI, accurate documentation and coding of chronic and acute conditions (especially risk-adjusting diagnoses such as HCC-relevant conditions) directly influence the risk profile assigned to a patient population. That risk profile is then applied when evaluating utilization and cost measures—such as total cost of care, inpatient admissions, ED use, and other resource consumption—so that providers or groups caring for more complex patients are not inappropriately labeled as inefficient simply because their patients require more services. This aligns with option B: risk adjustment is applied to resource utilization measures. Option A is incorrect because E&M levels are a professional billing construct and are not the basis for risk score calculation. Option C is incorrect because physician time may affect E&M selection under certain rules, but it is not the mechanism for risk adjustment in cost efficiency analytics. Option D is incorrect because procedures/interventions describe services rendered, not the adjustment methodology itself.
